The sidebar contains links to forms which you may need to fill out. In addition to registration and medical history there also form to transfer medical records to or from Cool Springs Pediatrics. If an adult other than the patient's parent or guardian is bringing them to the office, a consent to treat form should be filled out prior to the visit. These are also good to have on hand if the parents are travelling without the child or vice versa. The forms are pdfs therefore, you will need Adobe Reader to open and print the forms or consider the smaller and faster Foxit Reader. Both are free downloads.
